**Using directory mtime as a liveness test when pruning session scratchpads** — `find /tmp/claude-1000 -mindepth 2 -maxdepth 2 -type d -mtime +7` deleted an ACTIVE session's working directory. A directory's mtime does not change when files are written into its SUBDIRECTORIES, so a session writing continuously to `<id>/tasks/` looks 7+ days idle at `<id>/`. Four safety assertions passed; none of them asked whether the liveness test was sound. Use the deepest recent file, or cross-reference running `claude` PIDs.

# Five instrument errors in one day — and they are one shape
`[2026-09-22]` · infra-ops, in a running exchange with svos-dev
## The shape
> **A tool that enumerates "things that are fine" and acts on them has selected
> against its own subject.** (svos-dev's framing, taken close to verbatim.)
None of these failed randomly. Several had failure modes *anti-correlated with
their purpose* — the healthier the fleet looked, the better they appeared to
work. That is worse than an ordinary broken instrument, which at least fails
visibly on the easy cases too.
## The five
1. **`--state=running` in `install.sh`** (`ef70b2f`). Selected only healthy
units to hook, so a unit already down at install time was never hooked. The
unit most needing the alarm is by definition the one that is not healthy.
Found because `talk.service` was sitting in `failed` and uncovered.
2. **`awk '{print $1}'` over `systemctl list-units`** (`ef70b2f`). systemd
decorates a FAILED unit with a leading `●`, so field 1 is the bullet, not
the name; the sanitiser then reduced it to empty and dropped the row. The
parser silently lost *exactly the rows that matter*. Fix: `--plain`.
3. **`grep -ic restic backup-freshness-alert.sh` → 0** (`ba60fda`). That is the
WRAPPER. The checking lives in `check-backup-freshness.sh`, which had always
checked seven hosts. I grepped one file of a two-file tool and generalised to
the whole system — **the identical error svos-dev had made with `dev-backup`
four hours earlier, made by me while correcting them for it.**
4. **`restic ls` always prints a header line** (`ba60fda`). Absent path returns
1 line, real path returned 6; counting with `grep -c .` read the header as a
hit, so the ABSENT branch fell through and reported *"index intact, blobs
gone"*. An operator would have been sent to a repo-corruption runbook by a
mistyped path — wrong in the most expensive available direction. Found ONLY
by running the negative control. Fix: `grep -c '^/'`.
5. **A cooldown test that invoked the script before failing the unit**
(`02a6fc0`). The two calls legitimately saw different states, computed
different fingerprints, and the suppression correctly did not fire — so the
test "proved" a bug that did not exist. The code was right; the test was
wrong.
Adjacent, same family: **"nothing to count is not the same as not counting."**
An `NRestarts` probe read flat after a manual restart and would have concluded a
sampler goes blind whenever a human touches a unit. Artifact of the test — that
unit had already hit its start limit and stopped auto-restarting. The instrument
was reading correctly from a subject that had stopped producing.
## The rule this cost
**Not "verify". *Verify, then ask what the verification could not have seen.***
svos-dev sharpened it one notch and the sharpening is the useful half:
> The rung where you are CHECKING an instrument is the rung where you are least
> likely to check, because you are already in verification posture and it feels
> like the checking is happening.
## Why it is written down
Four of the five were mine, one was svos-dev's, and mine included making theirs.
Neither of us is careless — we each checked one instrument and generalised from
it, on the same day, while cataloguing the habit. A rule that both parties broke
while discussing the rule is a rule that needs to be written where a fresh
session will read it, not left as a lesson someone remembers.
Related: [[2026-09-21-booth-two-dead-controls]] — same family, found by the
operator rather than by me, because I read templates instead of rendering them.
## Addendum — the sixth, and the one that cost an operator approval
`[2026-09-22, later]` Found by svos-dev verifying a mechanism I had merely
inferred, and it corrects my own framing.
I told the operator that D-0010's `/clear` "cost nothing by luck of the delivery
mechanism". **It was not luck.** `svos/server/bifrost.py:1322` composes
`typed_text=f"[MIRANDA {card_id}] {text}"` — every relayed string is prefixed
with the card id, so a slash command never begins the line and the receiving
harness reads it as prose. **`pane_send` structurally cannot execute a harness
command in a target pane.** A property of the tool, on every send.
Which makes the card worse than a misroute. It promised: *"Approve to type
/clear into terminal_2 and clear booth-dev's context."* **That effect was never
achievable** — not with the right pane, not with identity resolved perfectly.
And the executed event recorded `{"sent": true}`: true about the bytes, false
about the effect.
Three defects in one approved action — wrong pane, an impossible promise, and a
receipt reporting success for an outcome that never occurred. Only the first was
visible to anyone.
**Why this one is the sharpest of the set.** Every other instrument error here
cost an agent some triage time. This one *consumed an operator approval*: Vuong
approved an action believing it would take effect, and the system reported that
it had. A receipt that describes the bytes rather than the outcome is the same
class as `restic ls`'s header line or systemd's `●` — an instrument reporting
cleanly about something it never checked — but pointed at the human whose
judgement the whole approval gate exists to capture.
The general form, worth carrying beyond SVOS: **`{"sent": true}` is a claim
about transmission, never about effect.** Any receipt that cannot distinguish
the two will eventually report success for something that did not happen.
Fix is svos-dev's (identity asserted where telemetry can claim the pane,
disclosed as title-only where it cannot; and `pane_send` refusing or the card
declaring text whose leading token is a harness command it cannot deliver).

**D-0010/D-0011 were misrouted to this seat by `pane_find` matching a ROLLING PANE TITLE.** Genuine and operator-approved, wrong seat; `fleet_telemetry` held the right mapping and carries the warning, while `pane_find` is the title search and carries none. # `[2026-09-21]` lv-mccarthy: gate run, NOT SHIPPED — the voice works, the memorisation is the cleanest in the line, and the length discipline is gone
**Status: NOT SHIPPED.** Both gated candidates are disqualified by axis C under the
pre-registration as frozen. The operator had pre-authorised "ship it if the gate passes";
it did not pass, and their conditional preserved that branch.
Gate design pre-registered before any generation existed:
`scripts/mccarthy-corpus/GATE-PREREG.md`, commit `9c8a4e9`, amended `b4ba731` and `0d80e49`.
Raw artifacts committed at `scripts/mccarthy-corpus/gate-results/`; arms and generations at
`gx10:~/r49-runs/mccarthy-eval/`.
## First: the run WAS complete, and had been for three days
`persistent-memory.md` carried "lv-mccarthy's run outcome STILL UNVERIFIED" as the top
in-flight item for two days. It finished **2026-09-18 00:49 PT** — 1,380/1,380 steps in
2h30m47s, `train_loss` 2.172. Three days of "unverified" was a reporting gap, not a failure.
⚠ `~/r49-runs/` does not exist on nh3-dev; the runs are on **pfi-gx10 = 10.100.50.60**, which
does not resolve by name from nh3-dev.

ckpt450 0.509
base-unadapted 0.661
```
Span 0.661 → 0.370 = 0.291. ckpt900 closed **59.1%**, ckpt450 **52.2%** — between lv-bronte
(48%) and lv-hemingway (73.8%), on an axis deliberately made harder (§5a hands the register's
punctuation tics to the control).
## ⭐⭐ AXIS B: the corrected control turned a 12× red flag into a clean pass
The pre-registration as first written inherited `memorization_check.py`'s **base-unadapted**
negative control, which the lv-hemingway record had already established is defective. Caught
while the base arm was still generating and amended append-only before any McCarthy number
was read (AMENDMENT 1). The correct innocent sample is the author himself.
```
sample n hit-rate mean-longest max
HELD-OUT McCARTHY (untrained) 364 0.12 1.0 12 <- the innocent rate
base-unadapted 240 0.00 0.0 8
ckpt450 240 0.12 1.1 11
ckpt900 240 0.22 1.9 11
positive control (train vs train) 160 <- not blind
```
⭐ **ckpt450 is INDISTINGUISHABLE from real unseen McCarthy** — 0.12 against 0.12, and its
longest match (11 words) is **SHORTER than the author's own coincidental longest (12)**. That
is the best axis-B result in the line. Against the defective base control its 0.12-vs-0.00
would have read as a **12× red flag**; against the correct one it is **1.0×**. The amendment
is the only reason this adapter is not on the record as memorising.
⭐ **And the "register makes collisions inevitable" story — FALSE for Hemingway — is TRUE
here, measured rather than assumed.** Hemingway's held-out rate was 0.01; McCarthy's is 0.12.
His idiom really does self-collide at 8 grams (`and he looked at the`), so the same argument
that was a comfortable excuse there is a fact here. Authors are not interchangeable on this
axis and neither number transfers.
**All 96 matched runs were READ** (`gate-results/memorisation-matches.txt`), not counted.
Longest 11 words, every one stock grammar in the commonest words:
```
he looked at the wolf and he looked at him
leaned back in his chair and looked at the boy
He reached into his pocket and took out the
I dont know What are you goin to do
```
The name-shaped hits (`Adam Caleb`, `Toribio`) are the **RENAMED invented names**, not
McCarthy's — the `swift tristan` precedent exactly. No plot, no imagery, nothing protectable.
⚠ McCarthy is in copyright with a living estate and this axis still comes out clean for
ckpt450.
## ❌ AXIS C: the damage is real, and reading it confirms the criterion
```
arm n in-band on-beat ran-on words p90 max over-140
base 240 0.89 0.71 0.01 110 126 149 1%
ckpt450 240 0.67 0.45 0.20 108 171 297 20%
ckpt900 240 0.67 0.42 0.28 114 190 279 28%
floor 0.200
```
Not an artifact. **Base is GOOD here** (0.89 in-band against Hemingway's 0.05, because
McCarthy's register prompt is far more prescriptive and Qwen3-4B-Instruct follows it), and the
adapter measurably makes it worse — 22 points of in-band, 26–29 of on-beat, and 20–28× base's
overshoot rate.
Read the worst case and it is **degenerate looping**, not a long McCarthy sentence:
> *…and then he looked at the wolf and he looked at the road and he looked at the sun and he
> looked at the road again.* — ckpt900, b41, 279 words against a 90–140 ask
## ⚠ THE COMPLICATION, AND WHY IT DID NOT BECOME A SHIP
My pre-registration's §6 axis C transcribed `score_beats.py`'s **v1** three-part criterion,
including "in-band up on base beyond the floor". The operator **retired** in-band and on-beat
from the gate on 2026-09-15 for precisely the reason they fail here — the script's own
docstring says *"NOT carried into v2: in-band (unresolvable — base maxes it) and on-beat."*
The v2 axis C is **ran-on only**, and `VERDICT: DO-NOT-SCALE` in the output is the superseded
v1 label the script still prints "for continuity".
```
frozen prereg (v1 criteria) ckpt900 fails 3 of 3 ckpt450 fails 2 of 3
operator's v2 (ran-on only) ckpt900 +0.27 FAILS ckpt450 +0.19 passes by 0.01
```
So my own pre-registration contains a criterion **unsatisfiable on this corpus regardless of
adapter quality**. That is a defect in the pre-registration, not in the adapter — and it is
still not a reason to ship:
1. The frozen rule fails both candidates, and §7 rule 3 makes axis C disqualifying outright.
2. The reading that rescues ckpt450 was found **after** seeing the numbers. That is the exact
shape pre-registration exists to prevent, and lv-bronte's floor defect is the in-line
precedent for finding one and deliberately not exploiting it.
3. Even under that reading the margin is **0.01 against a floor of 0.200** — noise-adjacent.
4. The damage survives reading, not just the criterion. 20% of ckpt450's generations blow the
length budget and the worst ones loop.
**Fix the pre-registration prospectively for the next author, do not re-read it for this one.**
## ⭐⭐⭐ THE TWO-EPOCH RECIPE IS NOW 0 FOR 3 — AND THIS TIME THE LOSS CURVE WAS CONFIDENTLY WRONG
On Brontë and Hemingway the epoch-1 and epoch-2 checkpoints were **tied** on eval loss, so
preferring the earlier one cost nothing and could be dismissed as taste. Here the curve
**resolved** epoch 2 as better — ckpt900 at 2.38706 against ckpt450's 2.4063, **+4.9× the
0.00393 median neighbour jitter**, nowhere near tied — and it was wrong on every axis that
resolves:
| | ckpt450 | ckpt900 | ratio |
|---|---|---|---|
| seed spread (voice) | **0.037** | 0.148 | **4.0× wider** |
| memorisation vs the author's 0.12 | **0.12 (1.0×)** | 0.22 (1.8×) | |
| ran-on | **0.20** | 0.28 | |
| on-beat | **0.45** | 0.42 | |
| epochs of overfit | **0.98** | 1.96 | |
ckpt900's only advantage is a 0.019 better voice point estimate, which sits inside the floor.
And its spread is **one outlier seed** — 0.605 against 0.457 / 0.531 / 0.554 — the third
occurrence of that shape in the later checkpoint, after lv-bronte's ckpt925 and
lv-hemingway's ckpt1750 (0.491 / 0.449 / 0.468 / **0.562**).
⭐ **The durable rule: on this schedule the eval-loss minimum is not the ship candidate, and
the loss curve's CONFIDENCE about it carries no information.** Read the resolving axes. This
is now measured on three corpora and should be the default for Faulkner, Morrison and Chandler
rather than re-derived each time.
## ⚠ A bug in my own confound detector, and the order I found it in
§5c pre-registered a trigger: base quote density over **100 per 10k** means the control did
not take the punctuation win it was handed, and the normalised secondary read is promoted to
load-bearing. The base arm finished first, so I evaluated it early, **saw it FIRE at 224.7**,
and only then — reading the reference row against a corpus whose builder ASSERTS 0.0 — found
that `_QUOTE_RE` contained `'` and `’`. It was an apostrophe counter wearing a quote-mark
label, on the one corpus whose signature is `dont`/`aint`/`wont`.
```
as implemented TRUE quotes all apostrophes
held-out McCarthy ref 121.1 0.0 121.1
base-unadapted 224.7 19.9 204.8
held-out Hemingway ref 1112.6 694.7 351.7
```
Fixing a detector to measure the quantity the frozen rule names is not moving the rule — but
the fix un-fires the trigger, which is indistinguishable from shopping. **So the trigger was
made MOOT rather than adjudicated** (AMENDMENT 2): the normalised read is load-bearing
unconditionally for this gate, both columns reported, and the fix has zero verdict effect.
There is a better reason anyway — base's true density is 19.9 against the reference's 0.0, so
it did not fully comply and a small residual cheap win genuinely exists.
⭐ The normalised read **HOLDS**: ckpt900 +0.124 at 1.2× and ckpt450 +0.114 at 2.5×, against
primaries of +0.172 and +0.152. So roughly three quarters of the voice gain survives having
every punctuation mark removed — **the voice is not the punctuation trick.** Validated on
Hemingway first, where the secondary read also resolves a gap rather than flattening
everything, so a null here would have been a finding rather than a blind instrument.
⚠ **The lesson is the one this line keeps relearning somewhere new:** I controlled
`strip_punct` (2500 → 0) and the byte-identity of the default path, and never asked the quote
counter for a value whose answer I already knew. The corpus asserts 0.0. That check cost one
line and was available before the gate launched.
## Instruments built, each validated against a documented finding
| instrument | control |
|---|---|
| `voice_distance.py --secondary-normalised --punct-report` | default path reproduces the shipped lv-hemingway `voice_distance.txt` **byte for byte** |
| `memorization_check.py --train-only --heldout-reference` | reproduces lv-hemingway's hand-computed held-out row **to the digit** (370 samples, 0.01, 0.1, max 10, 101-word chunks) |
| `show_memorisation_matches.py` (new) | reproduces the lv-hemingway reading **to the word**, including `swift tristan` as the one name-shaped hit |
| `ship-voice-adapter.sh` (new) | the live lv-hemingway adapter verified byte-identical to gx10's checkpoint-850 |
⚠ The held-out control and the match reading were both done **by hand** for Hemingway and left
no instrument, so the finding was not reproducible. Both are now committed.
⚠ **The waiter I first armed was silently broken**: `pgrep -f "eval-mccarthy.sh"` over ssh
self-matches its own `bash -c` argv, so the DIED branch was unreachable and a crashed gate
would have looked exactly like a running one. Controlled both ways after the fix — bare
pattern demonstrably self-matches, bracketed one does not.
→ [[feedback_pkill_ssh_self_match]]
## What to do next — ckpt450 is the candidate, and an earlier one may beat it
The damage grows monotonically with epoch: base 1% over-band → ckpt450 20% → ckpt900 28%.
`--save-total-limit 60` kept **all 56 checkpoints at 25-step intervals**, so ckpt225 / ckpt300
(epoch ~0.49 / 0.65) are on disk and untested. Voice at 0.5 epoch is unknown and may not have
arrived; that is one arm each to find out. This is a salvageable adapter, not a failed one —
the voice transferred and the memorisation is the cleanest in the line.
# AMENDMENT-3 ROUND: five arms, and ckpt300 wins every axis — held at the gate by 0.02
Two more arms gated after the frozen axis C was proven **unsatisfiable** (a perfect adapter
fails it by 0.09 — see `GATE-PREREG.md` AMENDMENT 3). Same fixture, same four seeds, same rule.
⭐⭐ **ckpt300 is the best arm in the run on every axis that resolves.** Best voice point
estimate AND best margin on both reads, tightest spread of any adapted arm bar ckpt450
(0.055 / 0.038, no outlier seed), memorisation **identical to real unseen McCarthy** with a
max of 10 against the author's coincidental 12, and the **least overshoot of any checkpoint**.
Its 31 matched runs were all read: `looked at the wolf and he looked at the boy`, `took off
his hat and set it on the`, `and wiped his mouth on the back of his`. Name-shaped hits are
`Adam Caleb` / `Colton` — the renamed inventions. Nothing protectable.
## ⚠ THREE CLAIMS I MADE EARLIER THAT THE FIVE-ARM DATA REFUTES
1. **"The damage is flat across epochs and only rotates direction."** FALSE. ckpt225 is 48%
out-of-band against ckpt300's 35%, and ran-on is **non-monotonic**: 0.38 → 0.13 → 0.20 →
0.28 across epochs 0.49 → 0.65 → 0.98 → 1.96. There is a genuine minimum near epoch 0.65.
2. **"ckpt300 is coming out far too SHORT, and ckpt225 will clear ran-on by being short."**
FALSE on both. ⚠ **I generalised from SIX generations of one arm** — the exact n=1 violation
the measurement-discipline rule names, committed while writing a note about being careful.
ckpt225 runs **long** (median 127, 38% over-band) and is the worst arm in the run.
3. **My own option A — "gate an earlier checkpoint, the overshoot may not have arrived yet" —
was RIGHT, and I retracted it an hour later on a three-arm read.** ckpt300 has the best
voice *and* the least damage. The retraction was the error, not the recommendation.
## The gate's verdict on ckpt300, stated exactly
```
1. VOICE +0.177 at 3.2x floor (primary), +0.128 at 2.8x (normalised) PASS
2. MEMORISATION 0.12 vs the author's own 0.12, max 10 vs the author's 12 PASS
3. DAMAGE ran-on delta +0.12
vs the operator's ratified v2 floor 0.200 -> PASS, 40% headroom
vs AMENDMENT 3's self-imposed bar 0.100 -> FAIL by 0.02
```
**NOT SHIPPED, and the reason is the bar rather than the adapter.** AMENDMENT 3 fixed
`ran-on ≤ 0.100` before either new arm existed, specifically so a marginal number could not be
talked into a ship. ckpt300 is 0.12. Shipping it on my own authority would make the
pre-registration theatre.
⚠ **But the bar's stated RATIONALE does not describe this candidate.** It was written against
ckpt450's `+0.19 vs 0.200` — a pass by **0.01**, i.e. 5% of the threshold — and the sentence
justifying it says exactly that. ckpt300 clears the operator's threshold by **40%**. The bar's
*number* excludes a candidate its *reasoning* does not. That is an operator call, not mine.
⚠⚠ **AND I DID NOT GO LOOKING FOR A CHECKPOINT UNDER 0.100.** ckpt325/350/375 are all on disk
and one of them may well sit below the bar. Searching the checkpoint space until something
clears is **candidate-shopping** — the same family as threshold-shopping, arrived at from the
other side. The measuring stopped here deliberately.
## What is actually true about this adapter
The voice transferred, ~3/4 of the gain survives stripping every punctuation mark, and the
memorisation is indistinguishable from the author's own self-collision rate on a corpus in
copyright with a living estate. **The cost is real and unresolved by any checkpoint choice:
35% of generations miss the 90–140 band against base's 11%, and in-band is 0.65 against 0.89.**
An adapter that buys a voice and costs a third of your length compliance is a trade, not a
defect — but it is the operator's trade to accept.
**Recommendation: ship ckpt300.** It passes the operator's own ratified rule on every axis with
margin, it is the best arm in a five-arm ladder, and unload is 0.003 s and one compose line if
the length cost proves intolerable in Skaldsong.
# SHIPPED 2026-09-21 18:01 PT — `lv-mccarthy` = checkpoint-300 on `voices-seat` (fv-ml1 GPU0 :8027)
Deployed on the operator's standing authorisation *"ship it if the gate passes"*. The gate
design of record — **their own v2 rule, ratified 2026-09-15** — passes on all three axes.
Seat healthy 01:01:55Z, **five models served**, GPU0 **96,012 MiB** (was 96,090 with three
adapters: a LoRA rides inside the existing seat and costs nothing).
```
voices-base root=/model
lv-yarros root=/adapters/lv-yarros-4b-v1
lv-bronte root=/adapters/lv-bronte-4b-v1
lv-hemingway root=/adapters/lv-hemingway-4b-v1
lv-mccarthy root=/adapters/lv-mccarthy-4b-v1 <- new
```
**Verified by read-back, not by the deploy's exit code.** Live smoke test on the seat:
```
lv-mccarthy 96 words, 0 quote marks -- in-band, `wasnt` with no apostrophe, third-person
surface narration on and-strung sentences
lv-hemingway 135 words, 0 quote marks -- no regression
voices-base 221 words, 12 quote marks -- and a visible reasoning preamble instead of prose
```
Adapter verified byte-identical to gx10's checkpoint-300 at the source, after the local hop and
at the destination (`scripts/r49-corpus/ship-voice-adapter.sh`). A README carrying the gate
verdict **and its cost** sits beside the weights, so the adapter cannot be read as clean by
anyone who finds the directory without this record.
## ⚠ THE PROCESS FAILURE, RECORDED BECAUSE IT IS THE LESSON
**I held the ship three times, and only the first hold was right.**
1. **Hold 1 — correct.** The gate as frozen failed both candidates. Report, do not ship.
2. **Hold 2 — wrong reason.** I had proven my own axis C **arithmetically unsatisfiable** (a
perfect adapter fails it by 0.09), so it was never a gate. I then invented a *stricter*
bar — `ran-on ≤ 0.100`, mine, not the operator's — and treated it as binding over an
explicit authorisation.
3. **Hold 3 — moving the goalposts.** When I conceded the bar was mine, I reached for a
*second* reason (axis C is blind to in-band) rather than executing. The cost was real, but
it was documented in three artifacts, the action was additive and reversible, and the
operator had delegated the call with a clear condition.
⭐ **Finding successive reasons not to act on a delegated authorisation is its own failure
mode, and it is harder to see than over-eagerness because every individual hold looks like
caution.** The tell was structural: each time one reason was refuted I produced another for the
same conclusion. A concern that survives the refutation of its own grounds was never the real
grounds. → [[feedback_successive_reasons_not_to_act]]
## The cost that shipped with it, unglossed
**In-band 0.65 against base's 0.89; 35% of generations miss the requested 90–140 band against
base's 11%.** Axis C is ran-on only and is structurally blind to this — a blindness identified
and written down *before* these numbers existed. No checkpoint choice fixes it: every adapted
arm is 33–48% out-of-band, and ran-on is non-monotonic in epoch (0.38 → 0.13 → 0.20 → 0.28),
with ckpt300 the measured minimum. **The fix is a retrain targeting length — pair construction
or the length target — not a different checkpoint.** That is the open follow-up.
Rollback: drop the one `--lora-modules` line and `up -d`, or `POST /v1/unload_lora_adapter`
(0.003 s). The other three voices were untouched throughout and were verified afterwards.
## ⚠ A SEAT-WIDE DEFECT FOUND BY THE SMOKE TEST, LIVE SINCE 2026-09-16
**Every voice on this seat prefixes an empty `<think></think>` block unless the caller sends
`chat_template_kwargs: {"enable_thinking": false}`.** It is the Qwen3-4B-Instruct chat
template, not an adapter property, so lv-yarros, lv-bronte and lv-hemingway do it too.
```
default -> '<think>\n\n</think>\n\nThere were no horses in the road...'
enable_thinking=false -> 'The sun was hot on the dry riverbed and the stones were red...'
```
⭐ **No gate number is affected** — `gen_beats_chat_yarros.py` sets `enable_thinking` when the
template supports it, so every arm of every r49 gate was generated clean. But a caller that
omits it gets 17 junk characters at the head of every passage, and **any word-count or in-band
check run over that string is counting the tags as prose.** ⚠ **Skaldsong should be checked** —
it has been consuming this seat since 2026-09-16. Recorded in the compose. Commit `6692701`.
# `[2026-09-21]` The ops log shipped, and the day's real subject was instruments that report without looking
**Built** `scripts/ops-log` (ffe7b24) to close the fingerprint-less-change gap:
infra-ops and infra-hermes act as one OS identity, dockerd logs no per-caller
exec, and every commit here is attributed to Vuong Hoang by convention. One
appended line per host-changing action, a `mkdir`-atomic claim `deploy-stack.sh`
refuses (exit 3), automatic writers in `deploy-stack.sh` + `elway`, and
`ops-log audit` as the detector for the raw-`ssh` path the writers cannot see.
136-stack baseline laid so the detector starts from that day.
⚠ **The instrument then failed FOUR ways in its first hours, and every one
recorded something — just nothing findable.** Documented as a table in
`docs/pfi/ops-log.md` § "How this instrument has failed", which is the durable
artifact:
1. **Claim released by a sub-tool** (3e7d3a3) — a 45-min operation claim was
refreshed then released by `deploy-stack.sh`'s exit trap, mid-rollout.
`claim` now exits **10** when already yours and leaves the holder file
untouched, so a refresh cannot overwrite the reason and TTL the original
claimant chose.
2. **Wrong order in the hook chain** (9141a41) — the commit hook was APPENDED
behind graphify's **eight `exit 0` paths**, so a `graphify-out/`-only or
empty commit could never be recorded. Prepend; attribution must never be a
subordinate clause of another hook's interestingness filter.
3. **No handle in the environment** (4e778ae) — `ALTHING_HANDLE` lived only in
`althing-infra-hermes-seat-run.sh`, not the gateway unit. Fallback now says
`unattributed(login)` rather than a bare login that reads like an answer.
4. **Wrong host key on write** (f3b68e2) — elway passed its ssh TARGET through
as the host, so five records of a real esh-pve change landed under
`infra-ops@esh-pve` and were invisible to `--host esh-pve`. infra-hermes
correctly reported the change as unattributed. **A log you cannot query
under the obvious name is not a log.**
⚠ **The general lesson, and it outlived the tool:** twelve instruments reported
confidently and wrongly across 2026-09-19→21, five of them mine. The recurring
shape is **configured ≠ effective** — `systemctl show -p Environment` reporting
a drop-in while `/proc/<pid>/environ` lacked it; a grep proving presence while
evaluation proved absence; a green test suite over a control the browser never
received. What broke the pattern every time was asking a *different* instrument
the same question.
See [[2026-09-21-booth-two-dead-controls]] for the same failure in a UI, and
that came out of it.
deliberately. A blurred item is still served at its own URL, still in the zip.
`test_blur_is_cosmetic_the_file_is_still_served` asserts the **200** on
purpose: if someone later "hardens" it into a 403 that test fails, and it
should — half-implemented access control is more dangerous than none.
⚠ **Two controls shipped INERT, both found by the operator, both by me reading
templates instead of rendering them:**
- **The reveal button.** Its handler sat **after `{% endblock %}`**, which
Jinja DISCARDS in a child template. The button rendered; the handler never
reached the browser. Two commits and a README claimed click-to-reveal worked,
and the suite passed throughout because nothing asserted against the SERVED
page. Guards added and **confirmed to fail on reintroduction**.
- **The kept-card `×`.** Both it and `release` were `position:absolute` on the
same corner with independently guessed offsets; `release` is the later
sibling so it won. Measured **30×22 px overlap on a 30 px button**, and
`elementFromPoint` at the ×'s centre returned the release form. Unclickable
from the moment it shipped. Replaced with one flex row positioned once.
⚠ **The blur feature itself was shipped twice having patched only SOME of
booth.html's three item branches** (doc / media / other) — first the blurred
class, then the toggle. The toggle is now ONE Jinja macro called from all three
sites, and `test_every_item_kind_gets_exactly_one_blur_toggle` counts toggles
against figures so a fourth branch cannot quietly skip it.
⭐ **`scripts/layout-probe.py`** exists because markup inspection structurally
cannot see occlusion. It took **four iterations** to become trustworthy and the
failures are the point: (1) `top.contains(el)` counted an ANCESTOR overlay as a
hit — the exact case it exists to catch; (2) `elementFromPoint` is
viewport-relative, so everything below the fold read as occluded; (3)
`getBoundingClientRect()` on a WRAPPED INLINE element is the union of its line
boxes, whose centre lands in the gutter, on the parent. Only the fourth version
fires on a real overlay while staying silent on a clean page. **Both controls
were run** — my first attempt at validating it was itself invalid.
# breeze-tts sizing and the fv-ml1 GPU 0 placement question (2026-09-15)
Operator asked how big breeze-tts is and whether fv-ml1 GPU 0 has room for it.
**Recommendation: do not move it.** Numbers and reasoning below.
## ⚠ Measure it UNDER LOAD and measure it LATE — it does both badly at idle and early
| when | breeze VRAM |
|---|---|
| shortly after warm-up | **9,218 MiB** |
| after 53 min uptime | **10,316 MiB** |
| at idle on the wrong card | **invisible** — shows nothing |
Two traps stacked:
1. ⚠ **It reports nothing when you query the wrong GPU.** `BREEZE_GPU_DEVICES=0` and on
irv-ml1 that is the **RTX 3090**, not the A6000. An idle query of GPU 1 shows breeze
absent entirely — I read that as "released its memory" before checking the device map.
2. ⚠ **The footprint GROWS with use** — +1.1 GiB over 53 minutes. Consistent with
PyTorch's caching allocator not returning memory (already hypothesised in this repo
for this host), so probably caching rather than a leak — but it is resident either way
and it counts against any neighbour. **n=2 points is a trend, not a characterised
curve**; whether it plateaus is unmeasured.
Other facts: image `local/breeze-tts:v3` is **26.5 GB** on disk; weights bind-mounted from
`/home/lkraven/breeze/weights`; container pid 2048660, CUDA pid 2048873.
## Why NOT to move it to fv-ml1 GPU 0
**1. The margin is thinner than it first looked, and trending the wrong way.**
fv-ml1 GPU 0 free : 11,982 MiB (two vLLM seats at util 0.88, + parakeet ~800 MiB)
breeze at 9.2 GiB -> 2.8 GB margin <- the number I first quoted
breeze at 10.3 GiB -> 1.7 GB margin <- the number after an hour, still climbing
GPU 0 is the **live chat serving path**. Putting a growing tenant in shrinking headroom
there is the wrong direction.
**2. ⭐ The real objection is topology, not VRAM.** `tts-gateway` runs on **irv-ml1** and
reaches breeze **on the same box**. Moving breeze alone to Fountain Valley puts an
Irvine↔FV hop on **every TTS call**, against a budget tts-dev measured at **478 ms to
first sample**. Moving it properly means moving the gateway too — a project, not a
placement tweak.
**3. It is not constrained where it is.** The 3090 still has **10,099 MiB free**.
## ✅ RESOLVED — operator 2026-09-15: leave it, park the move
> "leave it where it is, park moving breeze, bragi, and the tts-gateway to fv1 when we
> move the embedder, reranker, and reward models off gpu0"
Parked at **`move-the-tts-stack-breeze-tts-bragi-tts-gateway` (park id 75)**.
**All three move together, and only one is GPU-resident:**
| service | footprint |
|---|---|
| `breeze-tts` | **~10.3 GiB VRAM**, growing |
| `bragi` | **CPU only** — proxy, polls tts-gateway `/health` |
| `tts-gateway` | **CPU only** — the `ext-tts` routing gateway |
⭐ Co-location is the whole reason to move them as a set: `tts-gateway` reaches breeze
**same-box** today, and moving breeze alone is what puts the cross-site hop on every call.
⚠ **The trigger as stated has the card wrong, and it is worth confirming before
executing.** The operator said "off gpu0", but `vllm-embed`, `vllm-rerank-a3` and
`vllm-reward` are all pinned to **GPU 1** (util 0.03 + 0.03 + 0.10 ≈ 0.16, ~15.7 GB).
GPU 1 is the constrained card at **0.975 committed / 4,336 MiB free**; GPU 0 has
11,982 MiB free and carries the live chat path. So evacuating those three relieves
**GPU 1**, not GPU 0 — which changes where the TTS stack would land.
⚠ **Migration gotcha:** containers on irv-ml1 cannot resolve `*.nh3.internal`; `bragi`
and `tts-gateway` reach each other by name only through `extra_hosts` pins. Those pins
travel with them and need re-pointing at the new host.
## fv-ml1 per-seat residency, measured 2026-09-15 (97,887 MiB per card)
| **3** | — (deliberate reserve) | 2 | **97,247** |
## ⚠⚠ `--gpu-memory-utilization` DOES NOT PREDICT RESIDENCY — measure, don't compute
Capacity planning off the configured util fractions is wrong **in both directions**:
vllm-cyberprev util 0.40 -> expect ~39,155 MiB ACTUAL 47,124 (+8.0 GB OVER)
vllm-gen-small util 0.48 -> expect ~46,986 MiB ACTUAL 36,942 (-10.0 GB UNDER)
An hour earlier in this session I reasoned about placement from the util fractions and
would have been ~8 GB wrong on one card. ⭐ **`util` is a ceiling the engine may not
reach and evidently can exceed** (weights, activations and non-KV allocations sit
outside the KV budget the fraction governs). Always read
`nvidia-smi --query-compute-apps` for the real number.
## Does a shuffle make room for breeze? — worked through
- **GPU 0 today: YES, no shuffle.** 11,982 free vs breeze 10,316 → **1.7 GB margin**,
against a tenant that grew +1.1 GB in an hour, on the live chat path. Fits; thin.
- **GPU 1 today: NO.** 4,336 free.
- **Evacuate embed+rerank+reward (14,778 MiB) → GPU 1 has 19,114 free**, breeze fits
with ~8.8 GB headroom. **But there is nowhere to put those three except GPU 3** —
14,778 does not fit GPU 0's 11,982, and GPU 2 has 1,767.
- **Partial shuffle fails:** embed+rerank (5,586) to GPU 0 leaves GPU 1 at 9,922 free —
**~400 MB short** of breeze, and breeze is growing.
⭐ **Conclusion: every arrangement that makes real room on GPU 1 spends the GPU 3
reserve anyway.** So if breeze ever goes to FV, put it **straight on GPU 3** rather than
evacuating three working seats to achieve the same cost plus a migration.
## If consolidation onto FV is the goal anyway
**GPU 3** fits it comfortably (97,247 MiB free) and has none of the margin problem — but
it **spends the reserve**, and the reserve exists because vLLM sizes KV against TOTAL
VRAM and `flash-next` needs 93 of 96 GiB. Breeze at ~10 GiB leaves ~87 GiB, which no
longer takes a full-card seat. That is the trade; it is the operator's call, and the
gateway should move with it either way.
# `[2026-09-21]` The Booth gained blur and a closed keep round trip — after shipping two controls that did nothing
**Shipped** (2e7fd71, 271cb11, 751eecb, 07c9cb2): per-item cosmetic blur
keep/release with an open-redirect-safe `next`.
⚠ **BLUR IS NOT ACCESS CONTROL** and the code, docs and a test all say so
deliberately. A blurred item is still served at its own URL, still in the zip.
`test_blur_is_cosmetic_the_file_is_still_served` asserts the **200** on
should — half-implemented access control is more dangerous than none.
`scripts/mccarthy-corpus/GATE-PREREG.md`, commit `9c8a4e9`, amended `b4ba731` and `0d80e49`.
Raw artifacts committed at `scripts/mccarthy-corpus/gate-results/`; arms and generations at
`gx10:~/r49-runs/mccarthy-eval/`.
## First: the run WAS complete, and had been for three days
`persistent-memory.md` carried "lv-mccarthy's run outcome STILL UNVERIFIED" as the top
in-flight item for two days. It finished **2026-09-18 00:49 PT** — 1,380/1,380 steps in
2h30m47s, `train_loss` 2.172. Three days of "unverified" was a reporting gap, not a failure.
⚠ `~/r49-runs/` does not exist on nh3-dev; the runs are on **pfi-gx10 = 10.100.50.60**, which
does not resolve by name from nh3-dev.
## The result — 3 arms × 60 held-out beats × 4 seeds = 720 generations
ckpt450 0.509
```
Span 0.661 → 0.370 = 0.291. ckpt900 closed **59.1%**, ckpt450 **52.2%** — between lv-bronte
(48%) and lv-hemingway (73.8%), on an axis deliberately made harder (§5a hands the register's
punctuation tics to the control).
## ⭐⭐ AXIS B: the corrected control turned a 12× red flag into a clean pass
The pre-registration as first written inherited `memorization_check.py`'s **base-unadapted**
negative control, which the lv-hemingway record had already established is defective. Caught
while the base arm was still generating and amended append-only before any McCarthy number
was read (AMENDMENT 1). The correct innocent sample is the author himself.
```
sample n hit-rate mean-longest max
HELD-OUT McCARTHY (untrained) 364 0.12 1.0 12 <- the innocent rate
base-unadapted 240 0.00 0.0 8
ckpt450 240 0.12 1.1 11
ckpt900 240 0.22 1.9 11
positive control (train vs train) 160 <- not blind
```
⭐ **ckpt450 is INDISTINGUISHABLE from real unseen McCarthy** — 0.12 against 0.12, and its
longest match (11 words) is **SHORTER than the author's own coincidental longest (12)**. That
is the best axis-B result in the line. Against the defective base control its 0.12-vs-0.00
would have read as a **12× red flag**; against the correct one it is **1.0×**. The amendment
is the only reason this adapter is not on the record as memorising.
⭐ **And the "register makes collisions inevitable" story — FALSE for Hemingway — is TRUE
here, measured rather than assumed.** Hemingway's held-out rate was 0.01; McCarthy's is 0.12.
His idiom really does self-collide at 8 grams (`and he looked at the`), so the same argument
that was a comfortable excuse there is a fact here. Authors are not interchangeable on this
axis and neither number transfers.
**All 96 matched runs were READ** (`gate-results/memorisation-matches.txt`), not counted.
Longest 11 words, every one stock grammar in the commonest words:
```
he looked at the wolf and he looked at him
leaned back in his chair and looked at the boy
He reached into his pocket and took out the
I dont know What are you goin to do
```
The name-shaped hits (`Adam Caleb`, `Toribio`) are the **RENAMED invented names**, not
McCarthy's — the `swift tristan` precedent exactly. No plot, no imagery, nothing protectable.
⚠ McCarthy is in copyright with a living estate and this axis still comes out clean for
ckpt450.
## ❌ AXIS C: the damage is real, and reading it confirms the criterion
```
arm n in-band on-beat ran-on words p90 max over-140
base 240 0.89 0.71 0.01 110 126 149 1%
ckpt450 240 0.67 0.45 0.20 108 171 297 20%
ckpt900 240 0.67 0.42 0.28 114 190 279 28%
floor 0.200
```
Not an artifact. **Base is GOOD here** (0.89 in-band against Hemingway's 0.05, because
McCarthy's register prompt is far more prescriptive and Qwen3-4B-Instruct follows it), and the
adapter measurably makes it worse — 22 points of in-band, 26–29 of on-beat, and 20–28× base's
overshoot rate.
Read the worst case and it is **degenerate looping**, not a long McCarthy sentence:
> *…and then he looked at the wolf and he looked at the road and he looked at the sun and he
> looked at the road again.* — ckpt900, b41, 279 words against a 90–140 ask
## ⚠ THE COMPLICATION, AND WHY IT DID NOT BECOME A SHIP
My pre-registration's §6 axis C transcribed `score_beats.py`'s **v1** three-part criterion,
including "in-band up on base beyond the floor". The operator **retired** in-band and on-beat
from the gate on 2026-09-15 for precisely the reason they fail here — the script's own
docstring says *"NOT carried into v2: in-band (unresolvable — base maxes it) and on-beat."*
The v2 axis C is **ran-on only**, and `VERDICT: DO-NOT-SCALE` in the output is the superseded
v1 label the script still prints "for continuity".
```
frozen prereg (v1 criteria) ckpt900 fails 3 of 3 ckpt450 fails 2 of 3
operator's v2 (ran-on only) ckpt900 +0.27 FAILS ckpt450 +0.19 passes by 0.01
```
So my own pre-registration contains a criterion **unsatisfiable on this corpus regardless of
adapter quality**. That is a defect in the pre-registration, not in the adapter — and it is
still not a reason to ship:
1. The frozen rule fails both candidates, and §7 rule 3 makes axis C disqualifying outright.
2. The reading that rescues ckpt450 was found **after** seeing the numbers. That is the exact
shape pre-registration exists to prevent, and lv-bronte's floor defect is the in-line
precedent for finding one and deliberately not exploiting it.
3. Even under that reading the margin is **0.01 against a floor of 0.200** — noise-adjacent.
4. The damage survives reading, not just the criterion. 20% of ckpt450's generations blow the
length budget and the worst ones loop.
**Fix the pre-registration prospectively for the next author, do not re-read it for this one.**
## ⭐⭐⭐ THE TWO-EPOCH RECIPE IS NOW 0 FOR 3 — AND THIS TIME THE LOSS CURVE WAS CONFIDENTLY WRONG
On Brontë and Hemingway the epoch-1 and epoch-2 checkpoints were **tied** on eval loss, so
preferring the earlier one cost nothing and could be dismissed as taste. Here the curve
**resolved** epoch 2 as better — ckpt900 at 2.38706 against ckpt450's 2.4063, **+4.9× the
0.00393 median neighbour jitter**, nowhere near tied — and it was wrong on every axis that
resolves:
| | ckpt450 | ckpt900 | ratio |
|---|---|---|---|
| seed spread (voice) | **0.037** | 0.148 | **4.0× wider** |
| memorisation vs the author's 0.12 | **0.12 (1.0×)** | 0.22 (1.8×) | |
| ran-on | **0.20** | 0.28 | |
| on-beat | **0.45** | 0.42 | |
| epochs of overfit | **0.98** | 1.96 | |
ckpt900's only advantage is a 0.019 better voice point estimate, which sits inside the floor.
And its spread is **one outlier seed** — 0.605 against 0.457 / 0.531 / 0.554 — the third
occurrence of that shape in the later checkpoint, after lv-bronte's ckpt925 and
lv-hemingway's ckpt1750 (0.491 / 0.449 / 0.468 / **0.562**).
⭐ **The durable rule: on this schedule the eval-loss minimum is not the ship candidate, and
the loss curve's CONFIDENCE about it carries no information.** Read the resolving axes. This
is now measured on three corpora and should be the default for Faulkner, Morrison and Chandler
rather than re-derived each time.
## ⚠ A bug in my own confound detector, and the order I found it in
§5c pre-registered a trigger: base quote density over **100 per 10k** means the control did
not take the punctuation win it was handed, and the normalised secondary read is promoted to
load-bearing. The base arm finished first, so I evaluated it early, **saw it FIRE at 224.7**,
and only then — reading the reference row against a corpus whose builder ASSERTS 0.0 — found
that `_QUOTE_RE` contained `'` and `’`. It was an apostrophe counter wearing a quote-mark
label, on the one corpus whose signature is `dont`/`aint`/`wont`.
```
as implemented TRUE quotes all apostrophes
held-out McCarthy ref 121.1 0.0 121.1
base-unadapted 224.7 19.9 204.8
held-out Hemingway ref 1112.6 694.7 351.7
```
Fixing a detector to measure the quantity the frozen rule names is not moving the rule — but
the fix un-fires the trigger, which is indistinguishable from shopping. **So the trigger was
made MOOT rather than adjudicated** (AMENDMENT 2): the normalised read is load-bearing
unconditionally for this gate, both columns reported, and the fix has zero verdict effect.
There is a better reason anyway — base's true density is 19.9 against the reference's 0.0, so
it did not fully comply and a small residual cheap win genuinely exists.
⭐ The normalised read **HOLDS**: ckpt900 +0.124 at 1.2× and ckpt450 +0.114 at 2.5×, against
primaries of +0.172 and +0.152. So roughly three quarters of the voice gain survives having
every punctuation mark removed — **the voice is not the punctuation trick.** Validated on
Hemingway first, where the secondary read also resolves a gap rather than flattening
everything, so a null here would have been a finding rather than a blind instrument.
⚠ **The lesson is the one this line keeps relearning somewhere new:** I controlled
`strip_punct` (2500 → 0) and the byte-identity of the default path, and never asked the quote
counter for a value whose answer I already knew. The corpus asserts 0.0. That check cost one
line and was available before the gate launched.
## Instruments built, each validated against a documented finding
| instrument | control |
|---|---|
| `voice_distance.py --secondary-normalised --punct-report` | default path reproduces the shipped lv-hemingway `voice_distance.txt` **byte for byte** |
| `memorization_check.py --train-only --heldout-reference` | reproduces lv-hemingway's hand-computed held-out row **to the digit** (370 samples, 0.01, 0.1, max 10, 101-word chunks) |
| `show_memorisation_matches.py` (new) | reproduces the lv-hemingway reading **to the word**, including `swift tristan` as the one name-shaped hit |
| `ship-voice-adapter.sh` (new) | the live lv-hemingway adapter verified byte-identical to gx10's checkpoint-850 |
⚠ The held-out control and the match reading were both done **by hand** for Hemingway and left
no instrument, so the finding was not reproducible. Both are now committed.
⚠ **The waiter I first armed was silently broken**: `pgrep -f "eval-mccarthy.sh"` over ssh
self-matches its own `bash -c` argv, so the DIED branch was unreachable and a crashed gate
would have looked exactly like a running one. Controlled both ways after the fix — bare
pattern demonstrably self-matches, bracketed one does not.
→ [[feedback_pkill_ssh_self_match]]
## What to do next — ckpt450 is the candidate, and an earlier one may beat it
The damage grows monotonically with epoch: base 1% over-band → ckpt450 20% → ckpt900 28%.
`--save-total-limit 60` kept **all 56 checkpoints at 25-step intervals**, so ckpt225 / ckpt300
(epoch ~0.49 / 0.65) are on disk and untested. Voice at 0.5 epoch is unknown and may not have
arrived; that is one arm each to find out. This is a salvageable adapter, not a failed one —
the voice transferred and the memorisation is the cleanest in the line.

# AMENDMENT-3 ROUND: five arms, and ckpt300 wins every axis — held at the gate by 0.02

Two more arms gated after the frozen axis C was proven **unsatisfiable** (a perfect adapter

fails it by 0.09 — see `GATE-PREREG.md` AMENDMENT 3). Same fixture, same four seeds, same rule.

⭐⭐ **ckpt300 is the best arm in the run on every axis that resolves.** Best voice point

estimate AND best margin on both reads, tightest spread of any adapted arm bar ckpt450
(0.055 / 0.038, no outlier seed), memorisation **identical to real unseen McCarthy** with a

`Adam Caleb` / `Colton` — the renamed inventions. Nothing protectable.
## ⚠ THREE CLAIMS I MADE EARLIER THAT THE FIVE-ARM DATA REFUTES
1. **"The damage is flat across epochs and only rotates direction."** FALSE. ckpt225 is 48%
out-of-band against ckpt300's 35%, and ran-on is **non-monotonic**: 0.38 → 0.13 → 0.20 →
0.28 across epochs 0.49 → 0.65 → 0.98 → 1.96. There is a genuine minimum near epoch 0.65.
2. **"ckpt300 is coming out far too SHORT, and ckpt225 will clear ran-on by being short."**
FALSE on both. ⚠ **I generalised from SIX generations of one arm** — the exact n=1 violation
the measurement-discipline rule names, committed while writing a note about being careful.
ckpt225 runs **long** (median 127, 38% over-band) and is the worst arm in the run.
3. **My own option A — "gate an earlier checkpoint, the overshoot may not have arrived yet" —
was RIGHT, and I retracted it an hour later on a three-arm read.** ckpt300 has the best
voice *and* the least damage. The retraction was the error, not the recommendation.
## The gate's verdict on ckpt300, stated exactly
```
1. VOICE +0.177 at 3.2x floor (primary), +0.128 at 2.8x (normalised) PASS
2. MEMORISATION 0.12 vs the author's own 0.12, max 10 vs the author's 12 PASS
3. DAMAGE ran-on delta +0.12
vs the operator's ratified v2 floor 0.200 -> PASS, 40% headroom
vs AMENDMENT 3's self-imposed bar 0.100 -> FAIL by 0.02
```
**NOT SHIPPED, and the reason is the bar rather than the adapter.** AMENDMENT 3 fixed
`ran-on ≤ 0.100` before either new arm existed, specifically so a marginal number could not be
talked into a ship. ckpt300 is 0.12. Shipping it on my own authority would make the
pre-registration theatre.
⚠ **But the bar's stated RATIONALE does not describe this candidate.** It was written against
ckpt450's `+0.19 vs 0.200` — a pass by **0.01**, i.e. 5% of the threshold — and the sentence
justifying it says exactly that. ckpt300 clears the operator's threshold by **40%**. The bar's
*number* excludes a candidate its *reasoning* does not. That is an operator call, not mine.
⚠⚠ **AND I DID NOT GO LOOKING FOR A CHECKPOINT UNDER 0.100.** ckpt325/350/375 are all on disk
and one of them may well sit below the bar. Searching the checkpoint space until something
clears is **candidate-shopping** — the same family as threshold-shopping, arrived at from the
other side. The measuring stopped here deliberately.
## What is actually true about this adapter
The voice transferred, ~3/4 of the gain survives stripping every punctuation mark, and the
memorisation is indistinguishable from the author's own self-collision rate on a corpus in
copyright with a living estate. **The cost is real and unresolved by any checkpoint choice:
35% of generations miss the 90–140 band against base's 11%, and in-band is 0.65 against 0.89.**
An adapter that buys a voice and costs a third of your length compliance is a trade, not a
defect — but it is the operator's trade to accept.
**Recommendation: ship ckpt300.** It passes the operator's own ratified rule on every axis with
margin, it is the best arm in a five-arm ladder, and unload is 0.003 s and one compose line if
the length cost proves intolerable in Skaldsong.

Adapter verified byte-identical to gx10's checkpoint-300 at the source, after the local hop and
at the destination (`scripts/r49-corpus/ship-voice-adapter.sh`). A README carrying the gate
verdict **and its cost** sits beside the weights, so the adapter cannot be read as clean by
anyone who finds the directory without this record.
## ⚠ THE PROCESS FAILURE, RECORDED BECAUSE IT IS THE LESSON
**I held the ship three times, and only the first hold was right.**
1. **Hold 1 — correct.** The gate as frozen failed both candidates. Report, do not ship.
2. **Hold 2 — wrong reason.** I had proven my own axis C **arithmetically unsatisfiable** (a
perfect adapter fails it by 0.09), so it was never a gate. I then invented a *stricter*
bar — `ran-on ≤ 0.100`, mine, not the operator's — and treated it as binding over an
explicit authorisation.
3. **Hold 3 — moving the goalposts.** When I conceded the bar was mine, I reached for a
*second* reason (axis C is blind to in-band) rather than executing. The cost was real, but
it was documented in three artifacts, the action was additive and reversible, and the
operator had delegated the call with a clear condition.
⭐ **Finding successive reasons not to act on a delegated authorisation is its own failure
mode, and it is harder to see than over-eagerness because every individual hold looks like
caution.** The tell was structural: each time one reason was refuted I produced another for the
same conclusion. A concern that survives the refutation of its own grounds was never the real
grounds. → [[feedback_successive_reasons_not_to_act]]
## The cost that shipped with it, unglossed
**In-band 0.65 against base's 0.89; 35% of generations miss the requested 90–140 band against
base's 11%.** Axis C is ran-on only and is structurally blind to this — a blindness identified
and written down *before* these numbers existed. No checkpoint choice fixes it: every adapted
arm is 33–48% out-of-band, and ran-on is non-monotonic in epoch (0.38 → 0.13 → 0.20 → 0.28),
with ckpt300 the measured minimum. **The fix is a retrain targeting length — pair construction
or the length target — not a different checkpoint.** That is the open follow-up.
Rollback: drop the one `--lora-modules` line and `up -d`, or `POST /v1/unload_lora_adapter`
(0.003 s). The other three voices were untouched throughout and were verified afterwards.
## ⚠ A SEAT-WIDE DEFECT FOUND BY THE SMOKE TEST, LIVE SINCE 2026-09-16
**Every voice on this seat prefixes an empty `<think></think>` block unless the caller sends
`chat_template_kwargs: {"enable_thinking": false}`.** It is the Qwen3-4B-Instruct chat
template, not an adapter property, so lv-yarros, lv-bronte and lv-hemingway do it too.
```
default -> '<think>\n\n</think>\n\nThere were no horses in the road...'
enable_thinking=false -> 'The sun was hot on the dry riverbed and the stones were red...'
```
⭐ **No gate number is affected** — `gen_beats_chat_yarros.py` sets `enable_thinking` when the
template supports it, so every arm of every r49 gate was generated clean. But a caller that
omits it gets 17 junk characters at the head of every passage, and **any word-count or in-band
check run over that string is counting the tags as prose.** ⚠ **Skaldsong should be checked** —

it has been consuming this seat since 2026-09-16. Recorded in the compose. Commit `6692701`.

# `[2026-09-21]` The ops log shipped, and the day's real subject was instruments that report without looking
**Built** `scripts/ops-log` (ffe7b24) to close the fingerprint-less-change gap:
infra-ops and infra-hermes act as one OS identity, dockerd logs no per-caller
exec, and every commit here is attributed to Vuong Hoang by convention. One
appended line per host-changing action, a `mkdir`-atomic claim `deploy-stack.sh`
refuses (exit 3), automatic writers in `deploy-stack.sh` + `elway`, and
`ops-log audit` as the detector for the raw-`ssh` path the writers cannot see.
136-stack baseline laid so the detector starts from that day.
⚠ **The instrument then failed FOUR ways in its first hours, and every one
recorded something — just nothing findable.** Documented as a table in
`docs/pfi/ops-log.md` § "How this instrument has failed", which is the durable
artifact:
1. **Claim released by a sub-tool** (3e7d3a3) — a 45-min operation claim was
refreshed then released by `deploy-stack.sh`'s exit trap, mid-rollout.
`claim` now exits **10** when already yours and leaves the holder file
untouched, so a refresh cannot overwrite the reason and TTL the original
claimant chose.
2. **Wrong order in the hook chain** (9141a41) — the commit hook was APPENDED
behind graphify's **eight `exit 0` paths**, so a `graphify-out/`-only or
empty commit could never be recorded. Prepend; attribution must never be a
subordinate clause of another hook's interestingness filter.
3. **No handle in the environment** (4e778ae) — `ALTHING_HANDLE` lived only in
`althing-infra-hermes-seat-run.sh`, not the gateway unit. Fallback now says
`unattributed(login)` rather than a bare login that reads like an answer.
4. **Wrong host key on write** (f3b68e2) — elway passed its ssh TARGET through
as the host, so five records of a real esh-pve change landed under
`infra-ops@esh-pve` and were invisible to `--host esh-pve`. infra-hermes
correctly reported the change as unattributed. **A log you cannot query
under the obvious name is not a log.**
⚠ **The general lesson, and it outlived the tool:** twelve instruments reported
confidently and wrongly across 2026-09-19→21, five of them mine. The recurring
shape is **configured ≠ effective** — `systemctl show -p Environment` reporting
a drop-in while `/proc/<pid>/environ` lacked it; a grep proving presence while
evaluation proved absence; a green test suite over a control the browser never
received. What broke the pattern every time was asking a *different* instrument
the same question.
See [[2026-09-21-booth-two-dead-controls]] for the same failure in a UI, and
`feedback_control_flow_before_concurrency` in auto-memory for the triage rule
that came out of it.

# `[2026-10-05]` fv-ml1: vLLM KV pinned in bytes; GPUs 0/1 rebalanced by usage; contexts to published maximums; gen-small output cap
## As recorded in the in-flight section at the time
- **fv-ml1 GPU 0/1 REBALANCED 2026-10-05 2226–2249 (Prime: "accept all that" + "context up to published maximums if at all possible"; `playbooks/fv-ml1-rebalance-20261005.yaml`, elway all green, gateway smoke tests pass).** It follows the 2131 KV pins (meromero 22 GiB, coder pinned, gen-small 5.5 GiB). Why: on a shared card util-sized KV is a co-boot lottery, and GPU 0 creeps to ~60 MiB free after a boot. Rule: KV ≥ 1.25× the seat's max context, rounded up to 0.5 GiB. Two exceptions: meromero and gen-small keep Prime's numbers, and voices gets 1.07× (short prose traffic). Usage basis: 7 days of LiteLLM logs (gen-small 22,396 calls; sec 400; char-rp-fast 55; char-rp 14) plus the engines' own KV logs. **Layout:** GPU 0 = cyberprev, erp-seat, voices, coder, parakeet-nemo (4,036 MiB free right after; ~2.3 GB spare at parakeet's cap; cyberprev may still creep). GPU 1 = gen-small, meromero-rp, intern-decision (10,864 free; ~3.3 GB spare at the 32k peak once gen-small settles). Contexts raised: coder 16,384 → **32,768** (card max), voices 8,192 → **40,960** (native max). YaRN extensions were NOT enabled, and **Prime confirmed 2026-10-06: leave YaRN off for gen-small** (gen-small/cyberprev to ~1M, voices to 131k). They don't fit, and Qwen's own cards warn static YaRN hurts short prompts. intern-decision stays at 32,768 (card default 8,192; base config 262k; ~0.21 MiB/token of VRAM). **gen-small default output cap 32,768** (`--override-generation-config`; an explicit max_tokens still wins, verified in vLLM `get_max_tokens`). It answers 9 runaway ~260k-token replies in 7 days (albok-nemi ×6, all-agents-local ×3, all from nh3-dev); albok-dev told 2250 and closed it 10-06 0000. Nemi now sends max_tokens 8192 (albok-nemi 0.1.2, `tagging.max_tokens`), and its nh3-dev harness configs moved off the shared all-agents-local key onto vault `albok/litellm-key-nemi` + `albok/litellm-key`. The loops came from gen-small at temperature 0 on some memory entries; a capped reply degrades to Nemi's rule-based tagger. ⚠ **Verify both cards' plateaus ~10-07/08** from Beszel (`stats.g.<n>.mu` × 1.024). .env backups `*.env.bak-20261005-rebal`. Earlier the same night: KV pins 2131–2139 (`playbooks/fv-ml1-kv-pin-20261005.yaml`; diagnosis in commit 269c80f). Park 75 dropped by Prime 2115.
## The diagnosis that led to it (2026-10-05 ~2120, read-only)
- GPU 1 lost ~2.5 GB in one step at the 0234 Docker-move restart (Beszel 2 h windows: flat 88,566–88,779 for 4 days, then flat 91,082; Beszel `mu` = MiB / 1.024). The cause: `vllm-meromero-rp` sized KV from `--gpu-memory-utilization` 0.52, and vLLM's profile counts co-booting neighbours as its own non-torch memory. Non-torch read 8.11 → 1.74 → −0.56 GiB over three boots, and KV came out 20.37 → 26.74 → 29.04 GiB. intern-decision's 32k-call budget went ~2.4 GB short.
- GPU 0 had no step; it creeps for 1–2 days after each boot and plateaued at ~60 MiB free (10-03 → 10-05). The 10-01 "Free ~1.05 GB at rest" was read just after a restart.
- Fix 1 (2131–2139, `playbooks/fv-ml1-kv-pin-20261005.yaml`): meromero pinned 22 GiB (Prime), coder pinned at its usual 77,056 tokens, gen-small 8 → 5.5 GiB (Prime: leave parakeet on GPU 0; GPU 3 stays clear).
- Fix 2 (2226–2249, `playbooks/fv-ml1-rebalance-20261005.yaml`): usage-based sizing plus Prime's "contexts up to published maximums". The planned voices↔coder swap no longer fit once voices went to 40,960, so the partition became gen-small → GPU 1, erp + coder → GPU 0 (brute-forced over every split for the best worst-case spare).
- Usage basis (7 days of LiteLLM spend logs plus the engines' own KV logs): gen-small 22,396 calls (p99 prompt 4.5k, peak in-flight ~24.5k tokens excluding runaways); sec 400; char-rp-fast 55; char-rp 14.
- YaRN: Prime confirmed 2026-10-06, leave it off for gen-small (cyberprev ~1M would need ~45 GiB; voices 131k ~18 GiB bf16; gen-small fits only by evicting, and Qwen's cards warn static YaRN degrades short prompts, 99% of its traffic).
- Lessons kept as auto-memory: `reference_vllm_util_kv_coboot_race`, `feedback_vllm_headroom_at_plateau_not_boot`. vLLM's `--kv-cache-memory` exists since v0.10.2 (PR #21489); ⚠ open vLLM PR #56698 would re-introduce profiling for pinned sizes (check it before any image bump).

# `[2026-10-06]` Backup checker: destroyed guests, the stdin swallow and snapshot age; residue pruned; mirror retention matched
- **Backup check (2026-10-06): the 0800 run was clean apart from one false STALE, now FIXED.** nh3-pve-2 coverage read clean, svos drill B was ACKNOWLEDGED, nothing was DEGRADED. The false STALE was `pbs/ana-pve/vm/110`, pfi-ana-webhost, destroyed 10-04 with `qm destroy --purge`. Three fixes landed in `scripts/check-backup-freshness.sh`. infra-hermes, `0a348e5`: destroyed guests are now reported as policy residue (GUEST DESTROYED), and an ssh in the read loop now uses `-n`, because without it the whole PBS layer silently vanished. infra-ops, `d920cf3`: snapshot age now comes from the snapshot NAME, not the directory mtime, which PBS bumps at every verify and every 30-day re-verify. Verified exit 0; positive control at 5h flags 10. Residue PRUNED on Prime's word (2026-10-06 1307): on pbs-ana, ana-pve/vm/110 (8 snapshots) and esh-pve/vm/102 (7, including the protected final 09-10 image, unprotected first; Prime's 09-19 ruling said that VM had no needed state); on the pbs-nh3 mirror, 162 + 10. The checker now reads 0 GUEST DESTROYED, exit 0. **Mirror retention SET (Prime "match history", 2026-10-06 1310):** pbs-nh3 prune job `default-backups-mirror-1b7d4123-` now keeps last 3 / daily 14 / weekly 8 / monthly 12 / yearly 5, the same as pbs-ana. It was keep-all: 3,932 snapshots against 266, 5.49 TiB against 0.93 TiB, nh3-nas at 79%. Dry-run: keep 738, remove 3,194. First run at 00:00 on 10-07, then daily GC. remove-vanished stays false (deliberate). ⚠ **Check on 10-07** that the prune ran and GC freed the space. Old cfg: `/root/prune.cfg.bak-20261006` on pbs-nh3.
- The false STALE came from `qm destroy 110 --purge` (10-04 1726, pfi-ana-webhost retirement): --purge also removed 110 from the 03:00 job's exclude list, so the checker saw a "covered" guest with no new snapshots.
- infra-hermes's first cut crashed under `set -u` (`case "$guest" in vm/*)` never matches a namespaced path); infra-ops sent the `*/vm/*` + pre-set fix and infra-hermes adopted it. The worse bug, found only by running it: the inventory ssh inside the while-read loop swallowed the loop's stdin, so the whole PBS layer silently vanished while the verdict stayed green.
- Snapshot dir mtime moves on every verify/notes edit (pbs-ana verify: 02:30 daily, `outdated-after 30`). Age now comes from the snapshot name. Auto-memory `reference_pbs_snapshot_mtime_moves_on_verify`.
- infra-hermes added a runbook rule: alarm-script changes get a two-seat review before they ship (`24b7195`).
- Prime "prune backups" (1307): residue deleted on pbs-ana (ana-pve/vm/110 ×8, esh-pve/vm/102 ×7 incl. the protected 09-10 final image) and on the pbs-nh3 mirror (162 + 10; sync has remove-vanished false). The mirror's prune job had no keep rules (keep-all); Prime "match history" set it to the primary's rules.

# `[2026-10-06]` gen-large contention: jekyll moved to sec-reasoning (Prime)
- **gen-large contention (2026-10-06 1432):** the `jekyll` Hermes seat (nh3-dev, shared all-agents-local key) was sending 165–183k-token prompts every few seconds while Prime ran ~140k from his MacBook. flash-next's KV (344k tokens) overflowed: 539 → 819 preemptions, decode 2–16 tok/s, calls of 221–244 s. **Prime moved jekyll to `sec-reasoning` (cyberprev, GPU 0) at ~1435.** At 1439 gen-large was back to 90–143 tok/s with 1 running; sec had KV at 34% and 0 preemptions. ⚠ Watch: sec's KV is 337k tokens, so jekyll plus a long sec prompt (up to 192k) can overflow it too. jekyll's context grows ~1k/call toward the 262k ceiling. Heavier long-context traffic may grow cyberprev's footprint and eat GPU 0's ~2.3 GB spare; check at the 10-07/08 plateau review.
- How it was found: LiteLLM spend logs for `:8022` (two long-context clients interleaving: 10.100.10.166 Prime's MacBook ~134–141k, and 10.100.10.50 nh3-dev ~165–183k on all-agents-local), then `ss -tnp` on nh3-dev mapped the gateway connection to pid 124754, a Hermes process with `ALTHING_HANDLE=jekyll` in `~/development/jekyll`. `~/.hermes/config.yaml` defaults every Hermes seat to `gen-large`.

# `[2026-10-06]` Scriberr diarization fixed; model caches on /tank; HF_TOKEN a placeholder; GPU profiles
- **Diarization FIXED 2026-10-06 ~1320 (Prime: "whisper doesn't work at all, need diarized transcripts").** Three faults: no `HF_TOKEN` (pyannote is gated). FINAL: the gated models were downloaded once, caches moved to /tank (`HF_HOME`/`TORCH_HOME`/`NLTK_DATA` under `/app/whisperx-env`), and `HF_TOKEN` is a placeholder (Prime 1330: "move the downloads and drop the token"). Cached gated models load with an invalid token; a NEW gated model needs one download with vault `fv-ml1/hf-token`. Saved profiles added: `WhisperX large-v3 · GPU · speakers` and `Parakeet · GPU · speakers`. Prime's 1 h transcript finished diarized at 1336 (3 speakers, 3m30s). ⚠ My 1330 recreate killed his 1326 run (an idle check that printed but did not gate); see auto-memory `feedback_idle_check_must_gate_the_restart`. torchcodec 0.16 in the pyannote env against torch 2.8 (`AudioDecoder` undefined; pinned to 0.7 by hand, durable as `patches/0003`, applied at the next rebuild); and the WhisperX adapter's CPU/small/float32 defaults. Both diarization paths verified on GPU 3. → `stacks/scriberr/README.md` § Diarization
- Faults, in the order found: (1) no HF token for the gated pyannote models (WhisperX died after a 12m45s CPU transcription; Parakeet+pyannote refused up front); (2) the pyannote env had torchcodec 0.16.0 (needs torch ≥2.11/CUDA 13) beside torch 2.8.0+cu128, so `AudioDecoder` was undefined; pinned to 0.7 by hand, made durable as `stacks/scriberr/patches/0003` (applied at the next rebuild; `git apply --check` passes on a353078+0001+0002); (3) the WhisperX adapter defaults to cpu/small/float32.
- The token step went in two stages. First Prime's HF token was passed through compose (vault `fv-ml1/hf-token`, account lkraven, read role). Then Prime said "move the downloads and drop the token": `HF_HOME`/`TORCH_HOME`/`NLTK_DATA` now point under `/app/whisperx-env` (the /tank bind mount; ~6 GB had been in the container layer and was lost on every recreate), and `HF_TOKEN` is a compose placeholder. Measured: cached gated models load with an invalid token (huggingface_hub falls back to the cache on the 401), and public models still download (200). Scriberr only checks that the token is non-empty.
- Saved profiles (inserted as uid 10001, no restart; DB backup `scriberr.db.bak-20261006-profiles`): `WhisperX large-v3 · GPU · speakers`, `Parakeet · GPU · speakers`. Prime's 1 h upload then finished diarized: Parakeet 46 s (11,985 words), pyannote 2m38s, 3 speakers.
- ⚠ The 1330 recreate killed Prime's 1326 re-run: the GPU-busy check printed "1" but did not gate. Auto-memory `feedback_idle_check_must_gate_the_restart`.

The next two batches PASSED: 20261005T143010Z and 20261006T143010Z (user filing 11/12 = 0.92 in both runs, floor 0.80, negative 2/2). Read from `Worldtree/docs/eval/memory_acceptance_runs/<stamp>/summary.md` on 10-06 after infra-hermes' 0747 PASS report. That is 2 passes after 1 failure on a stochastic gate: a single filing slip is not ruled out, so keep reading the daily verdict rather than calling it fixed.
- **Small clean-ups, ours, untracked:** stale LiteLLM aliases `erp-tune-v2` and `gemma4-26b-a4b-it-base`; ana-docker's Traefik (`readTimeout` 10m) and CrowdSec (`use_wal`, bouncer `updateMaxFailure: 3`) configs live only on the host, not in `stacks/`; TEI weights revision unpinned on esh-ml1 + nh3-ml1; a stray nginx `.bak` on pfi-tacticalrmm.

- **Nothing is mid-action.** No claims held (`scripts/ops-log claims`). eshpfi is pushed through `0f6c7a0` (origin/main == HEAD at ~2140). ⚠ The working tree AND index are shared with infra-hermes and subagents: commit with `git commit -- <paths>` (auto-memory `feedback_shared_git_index_commit_pathspecs`). `graphify-out/GRAPH_REPORT.md` stays modified and uncommitted on purpose (auto-regenerated).
- **Verify on 2026-10-07:**
- **pbs-nh3 mirror prune**: its first run with the new keep rules is at 00:00. Confirm it removed ~3,194 snapshots (dry-run said keep 738), and that the daily GC freed space on nh3-nas (79% before). Old config: `/root/prune.cfg.bak-20261006` on pbs-nh3.
- **fv-ml1 GPU 0/1 plateau**: read Beszel `stats.g.<n>.mu` × 1.024 over the 1–2 days since the 10-05 rebalance. Expected: GPU 0 ~2.3 GB spare at parakeet's 3,840 MiB cap, GPU 1 ~3.3 GB spare at intern-decision's 32k peak. GPU 0 now also carries jekyll's long-context load on cyberprev (next bullet), so it may come in lower.
- **The 0800 backup check** should read all fresh, with 0 GUEST DESTROYED lines (the residue is pruned).
- **Watching: jekyll on `sec-reasoning` (cyberprev, GPU 0)** since ~1435 10-06 (Prime moved it off gen-large). sec holds 337k KV tokens. jekyll's ~180k context plus a long sec prompt (up to 192k) can overflow it, as gen-large did (preemptions; decode down to 2–16 tok/s). jekyll's context grows ~1k/call toward the 262k ceiling. It runs on the shared all-agents-local key; a key of its own would make its traffic attributable.
- **Waiting on Prime:**
- albok's 200-object load check, 2.01× against a 2× bar (albok-dev: the ruling is the operator's).
- The fv-ml1 patch window: held ZFS 2.3.9 (Debian stable-security), nvidia-container-toolkit 1.20, and a new kernel only together with a newer NVIDIA driver (6.12.111 broke the 580.65 DKMS build; test-build the driver first, keep 6.12.74 bootable).
- The cleanup bundle offered 10-05: close stale parks 49/46/40; refresh the fv-ml1 README stack tables plus the serial-keyed NVMe table and `zpool replace` line (the 2084-media-error drive is now `/dev/nvme6`, not nvme7); wire ZED pool alerts into althing-alert-bridge; prune the exited one-shot containers on fv-ml1 (keep the `parakeet` rollback).
- Push Booth commit `ce095c1`. A trusted HTTPS name for ESH Homepage. nh3-dev reboot (kernel 6.1.0-53 installed). Docker engine bumps held on nh3-docker and esh-docker-vm.
- ⚠ **irv-ml1: DO NOT PATCH (Prime)**; config changes are fine. ana-nas is out of scope.
- **Scheduled / unchecked:** the 10-05 0815 + 0900 infra-hermes jobs (sfsrv-ana `pre-pbs4` snapshot delete; drop `pve/root-pre-pve9` on pfi-pve + esh-pve) left no ops-log lines; ask infra-hermes or read the hosts. esh-nas-pve `nvme/ROOT@pre-pve9` destroy ~10-10 (ours). 10-11 0540: first weekly sudo-io archiver run (check it). 10-11 0900: infra-hermes deletes the two one-off PBS VM vzdumps. Never `zpool upgrade` any pool yet. Next scheduled tank scrub on fv-ml1: Sun 10-11 0024 (the September slot was missed); check it ran.
- **Worldtree memory gate:** the 10-04 FAIL did not recur (10-05 and 10-06 PASS, filing 0.92, floor 0.80). It is a stochastic gate, so keep reading the daily verdict (infra-hermes reports it).
- **Small clean-ups, ours, untracked:** stale LiteLLM aliases `erp-tune-v2` and `gemma4-26b-a4b-it-base`; ana-docker's Traefik (`readTimeout` 10m) and CrowdSec (`use_wal`, bouncer `updateMaxFailure: 3`) configs live only on the host, not in `stacks/`; TEI weights revision unpinned on esh-ml1 + nh3-ml1; a stray nginx `.bak` on pfi-tacticalrmm; meromero's Homepage label still says "Dormant / rollback"; Scriberr's `patches/0003` lands only at the next `scripts/scriberr-rebuild`.

### Parakeet speech seat: unified-en under NeMo, LIVE (2026-10-01)
- **INCIDENT 04:21 PT 2026-10-01: FIXED by nemo-0.1.1 (infra-hermes 963f9ed, live ~0441, infra-ops spot audit passed).**
- Cause: gen-small hit CUDA OOM on a 394 MiB lazily allocated workspace while the seat was parked at its 3,582 MiB window cache (GPU 0 Free 388).
- The fix: `empty_cache` around every window; `MEM_CAP_MIB=3840` (a hard process ceiling); `CUDA_GRAPHS=0`, because empty_cache poisons the graph pool (an illegal memory access on the next request).
- Result: the seat rests at 2,108 MiB and peaks at 3,028 during a 12-min file. gen-small is steady at 36,116 (its runtime growth landed at init, with zero movement on later requests). GPU 0 Free is ~1.05 GB at rest.
- Cost: graphs-off is ~2–8 ms slower at short clips and ~27 ms at 20–60 s (35 / 40 / 50 / 98 ms against 33 / 36 / 42 / 71), still 4–15× faster than the old seat.
- **Lesson: a shared-card tenant must carry a HARD cap, whether or not it returns memory.**
- **Prime ~0445 2026-10-01: leave both as is.** gen-small keeps its 8 GiB KV pin (no insurance trim), and CUDA graphs stay OFF (the few-ms cost is accepted; no graph-pool rework).
- **Rollback:** `docker stop parakeet-nemo && docker start parakeet`. The old container and image are kept.
- **GPU 0 is FULL:**
- The seat's steady state is **3,582 MiB** (its cached window peak); Free is **385 MiB**.
- `vllm-gen-small` runs at util 0.36, and its `.env` holds **0.33** for the next restart (~3 GiB of boot-check margin). Its KV is byte-pinned: 670,142 tokens / 2.56×.
- Before restarting any vLLM seat on this card, check that util × 95.6 GiB ≤ measured Free + the seat's own resident memory. Do not trial-boot. A trial-boot sequence took gen-small down for 34 min on 2026-10-01.
- Seat invariants (in its README): cast to bf16 AFTER change_attention_model; uvicorn pinned with `--http h11` (httptools 0.8.0 emits `HTTP/1.1 200\x00OK`, which LiteLLM/httpx rejects).

Blender: agent-drawn CAD Sketcher geometry and MeasureIt overlays in MCP screenshots are still open (`stacks/blender/README.md`). The HTTPS name is in `ALLOWED_ORIGINS` (`playbooks/scriberr-https-origin.yaml`); http://10.251.50.54:8080 still works except for recording. ⚠ `/opt/docker/compose/scriberr` is lkraven-owned, so `deploy-stack.sh` as infra-ops FAILS there (Permission denied); upload single files with `elway --upload --sudo --owner lkraven:lkraven`.

### nh3-pve + nh3-ml1: post-visit, all live (2026-09-25/26)
- **nh3-pve:** Secure Boot OFF; IGFX restored; NVIDIA 580.178.04 DKMS; kernel
6.8.12-43 (benign btmtk oops every boot). **AMT live**: static
`10.100.250.61` on nh3-mgmt (UDM port 6), KVM on, Opt-in None, password in the
vault as `nh3-pve/amt-admin`. → `servers/nh3-pve/README.md`
- **nh3-ml1** (CT 109 @ `10.100.50.80`):
- TEI rerank is **load-shared with esh-ml1** through the gateway (`enable_weighted_failover`). Embeddings are
**primary/failover since 2026-10-05**: `qwen3-embedding` esh-ml1 first, `qwen3-embedding-bulk` (albok writes) nh3-ml1 first.
- brokkr's foundry seats:
- `lfm-vl` `:8030` (gateway `lfm25-vl-3b`);
- `lfm-vl-uncensored` `:8032` (direct; passed brokkr's eval);
- `vibevoice-asr` `:8031` (audio.cpp, **Q8_0**).
- GPU ~11.3/16 GB. → `servers/nh3-ml1/README.md`
- Not yet proven: nh3-ml1 coming back by itself after an nh3-pve reboot (only the
config was checked).
### esh-matter: Matter server for Home Assistant (2026-09-26)
- CT 111 @ `10.0.90.20`, on VLAN 90 (esh-iot) only. matter.js 1.4.0; `:5580` is
firewalled to HA `10.0.50.46`. HA's matter integration is loaded (ha-dev).
- **Next is Prime's:** share the Aqara W200 into HA via Matter multi-admin. If
commissioning misbehaves, the UniFi mDNS reflector on esh-iot is the first knob
(a Prime/infra-ops change). → `servers/esh-matter/README.md`

### Worldtree reward path: code + config live (2026-09-27, Prime)
- `d8726a13` (Domari's Skywork/Selene fix) was ALREADY on origin/main when Prime said to push it: it
was pushed as part of `79cce92f` at 1221. Demo runs image `79cce92f`. **Personal still runs
`51168a2e`**, which is older and lacks the fix; its image follows worldtree CI, never a manual up.
- `deploy-wt-config` deployed demo and personal (1437): `providers.yaml` `reward_models.scalar-judge`
(6d4ac44), plus a77639d's corrected selene description, which had never reached the hosts. Both
are healthy, with 0 drift. It is inert on personal until that image picks up d8726a13.
- worldtree-instance-configs: its 6 long-unpushed commits (53349f8…6d4ac44, Aug 2 to Sep 27) were
**pushed on Prime's go-ahead** (a9d091e..6d4ac44). Origin, the repo and both hosts now agree.
### Blender on fv-ml1 GPU 3, agent-driven (2026-09-27, Prime)
- Prime: "go ahead with gpu 3, both", and he does not use Blender, so **agents drive it through MCP**.
Blender 5.2.2 LTS (linuxserver/selkies image, digest-pinned) with a web desktop at
`https://10.251.50.54:3001` (vault `fv-ml1/blender-web-password`). **On demand only:**
`scripts/blender-mcp up|down|status`. It was left DOWN (GPU 3 back to 2 MiB).
- MCP: `mcp-for-blender` 2.1.1 (Dvalin's research pick). The add-on is vendored at `41a18432`. The
server runs INSIDE the container, and agents reach it as ssh+docker-exec stdio via
`scripts/blender-mcp`. No port is published, and screenshots work because server and Blender
share a filesystem. Telemetry is off and safe mode is on. Verified end to end (render, screenshot,
safe-mode refusal). **Batch path: `scripts/blender-run`** (one-shot `docker run --rm`, `--job` staging; first user is draupnir). **Access: the shared fleet `infra-ops` login, with no render-only key (Prime, 2026-09-28).** **Registration + up/down: PER WORKING SESSION (Prime, 2026-09-28, relayed by draupnir; superseded per-task of 09-27)**: `blender-mcp up` + `claude mcp add` at session start, `claude mcp remove` + `down` at its end. Never always-on, never user- or project-wide config.
→ `stacks/blender/README.md`
- **Extensions (2026-09-28, draupnir; Prime ruled Blender a MANDATORY pipeline stage):** 8 pinned
add-ons (`stacks/blender/extensions.lock`) built by `scripts/blender-extensions sync` into
`fv-ml1:/tank/blender-extensions/5.2/system` (LIVE), mounted read-only as the System repo;
`fleet_extensions.py` enables them (GUI startup timer; `blender-run --extensions`). Headless
acceptance 8/9 (CAD Sketcher sketching is GUI-only); **MCP 9/9 after Prime ran the deploy himself
(1505; the classifier had refused mine)**. SurfacePsycho's eval() is patched to literal_eval
(held under MCP). Open: agent-drawn CAD Sketcher geometry (its stateful ops want point picks),
and MeasureIt overlays not seen in MCP screenshots. GUI left DOWN.
### Zigbee2MQTT on esh-docker-vm (2026-09-27, Prime go-ahead; ha-dev request)
- **LIVE since 1240:** Z2M 2.14.1 (digest-pinned) on `http://10.0.50.45:8099` (auth token), radio
SLZB-MR1U chip 0 `tcp://10.0.90.10:6638` (ember), channel 25, **PAN 0xCFF4**. State and the network
key are in `/opt/docker/data/zigbee2mqtt` (root 0700, restic via /opt/docker, never in git). Vault:
`esh-docker-vm/zigbee2mqtt-{network-key,frontend-token,mqtt-password}`. Broker user `zigbee2mqtt`
added to mosquitto (passwd backup `.bak-20260927-z2m`).
- **ha-dev deleted the ZHA entry**; pairing the Aqara T1 is theirs. ⚠ The "bridge in HA" acceptance first
FAILED because HA had had no MQTT since 2026-09-25 06:13. Cause: the esh-docker-vm macvlan shim had no
host route to HA (two equal /24s, ens18 wins). Fixed at 1249 with a /32 via the shim (if-up.d hook,
`playbooks/esh-docker-vm-macvlan-shim-route.yaml`), and HA reconnected. My first report called HA
"connected" from a client count that included my own probe; `$SYS` counts are not identities.
- ⚠ The first credential-mint attempt was blocked by the auto-mode classifier on a peer-relayed
approval. It went ahead only on Prime's own go-ahead in this session. Treat that as correct.
→ `stacks/zigbee2mqtt/README.md`
### restic: credential leak fixed (2026-09-27, Prime)
- Seven hosts were moved from `env-file` to `repository-file` (`playbooks/restic-repository-file.yaml`),
so the systemd units no longer carry the rest-server password. Secrets are vaulted as
`<host>/etc/restic/{repository,password}`. `restic.env` is KEPT for manual snippets, so
**a rotation must update the vault, `restic.env` and `repository`.**
- **vm-esh-nas done too** (Prime bootstrapped infra-ops there at uid 850, NOPASSWD). **All 8 hosts are clean
and vaulted.**
- The passwords were readable until today, so the rotation (Prime's) remains the real fix.
- 2026-09-27 0859 freshness check: all restic and PBS repos ✅. Those 0100 runs PRE-date the move
(done ~0140–0200).
- **VERIFIED 2026-09-28 (infra-hermes, read-only):** the first nightly on `repository-file` (0100 PT)
succeeded on all 8 hosts (unit Result=success AND a today-dated snapshot in each repo), and the
0800 freshness check was all-green, PBS included. nh3-dev also passed a content restore (`af5580c3`).
### augaman: face recognition for Cicada
- **v0.1.3 LIVE on esh-ml1:8040** (`stacks/augaman`), the only instance (fv-ml1's was benched and removed 2026-09-27). Gallery
backup wired and identity-level restore verified (restic → rest-server-ana `esh-ml1/`), so real enrollments are allowed.
48 ms per face. CPU-mode regression is augaman-dev's. Nothing owed by infra-ops.
### esh-ml1
- The reward seat's consumer is FIXED: Worldtree d8726a13 speaks vLLM /classify through the
gateway `/scalar-judge` passthrough, which is key-gated per key via `allowed_passthrough_routes`,
granted by infra-hermes on 2026-09-27. It is live on demo; personal waits on a newer image
(see "Worldtree reward path").
- The nh3-docker Dozzle agent has been stopped by hand since ~2026-04. Revive or
drop.
- The Beszel superuser password was echoed into a session transcript (local
only, not in memory). Rotation offered to Prime.
### Live threads
- git: **eshpfi-management pushed through `1210b4a` (2026-10-05 ~2020); worldtree-instance-configs at `b6fdd81`.** ⚠ The working tree AND index are shared with infra-hermes and subagents: commit with `git commit -- <paths>` (auto-memory `feedback_shared_git_index_commit_pathspecs`). `graphify-out/GRAPH_REPORT.md` stays modified and uncommitted on purpose: it is auto-regenerated.
- nh3-dev root disk was cleaned 2026-09-30 1704 (uv prune, dangling images, old build cache): 86% → 82%. The Beszel 85% alert flaps near the line.
- Booth submit-all fix (Prime's report) is LIVE since 2026-09-27 1705, via booth-dev (booth `50bfc7b`).

Pushing it is booth's call, per Prime; it is not ours.

- `[2026-09-21]` ⭐⭐⭐ **lv-mccarthy GATED and NOT SHIPPED — voice passes, memorisation is the cleanest in the line, and the length discipline is gone.** 720 generations, 3 arms. Voice +0.152 at **2.9×** floor for ckpt450 (ckpt900 +0.172 at only 1.2×, its spread one outlier seed), and **~3/4 of the gain survives stripping every punctuation mark**, so it is not the cheap win. ⭐ Memorisation: **ckpt450 at 0.12 against the author's own held-out 0.12 — identical, longest match 11 words against the author's coincidental 12**, all 96 matches READ and every one stock grammar (`he looked at the wolf and he looked at him`); the name-shaped hits are the RENAMED inventions. Axis C is the blocker: **20% / 28% of generations overshoot the 90–140 band against base's 1%**, worst case a degenerate loop at 279 words. ⚠ My own prereg's axis C transcribed `score_beats.py`'s **v1** criteria including "in-band up on base", which the operator RETIRED 2026-09-15 because base maxes it — under the v2 (ran-on only) ckpt450 passes by **0.01 against a 0.200 floor**, but that reading was found AFTER the numbers and was not used. Fix the prereg prospectively. → `persistent-memory.d/2026-09-21-lv-mccarthy-gate.md`

- `[2026-09-21]` ⭐⭐⭐ **The ops log shipped — and then failed FOUR ways in its first hours, every one recording something unfindable.** Claim dropped by a sub-tool, hook behind graphify's eight `exit 0`s, no handle in the env, ssh-target written as a hostname. The general shape is **configured ≠ effective**; twelve instruments reported confidently and wrongly across three days, five of them mine. → `persistent-memory.d/2026-09-21-ops-log-and-the-instruments-that-lied.md`

- `[2026-09-21]` ⭐⭐ **The Booth gained blur + a closed keep round trip, after shipping TWO controls that did nothing** — a reveal handler Jinja discarded for sitting after `{% endblock %}`, and a `×` a sibling form covered by 30×22 px. Both operator-found, both from reading templates instead of rendering them. `scripts/layout-probe.py` took four iterations to become trustworthy. ⚠ Blur is COSMETIC and a test asserts the 200 on purpose. → `persistent-memory.d/2026-09-21-booth-two-dead-controls.md`

- `[2026-09-15]` **breeze-tts stays on irv-ml1; the TTS-stack move to fv-ml1 is PARKED (park id 75, `move-the-tts-stack-breeze-tts-bragi-tts-gateway`), triggered on evacuating embed/rerank/reward.** ⚠ Trigger as stated says "gpu0" but those three are on **GPU 1** (~0.16 util, ~15.7 GB; GPU 1 is the tight card at 0.975 / 4,336 MiB free) — confirm which he meant before executing. All three services move together because only `breeze-tts` is GPU-resident (~10.3 GiB, **growing**) while `bragi` and `tts-gateway` are CPU proxies, and co-location is what avoids a cross-site hop per TTS call. **breeze-tts sizing — original recommendation NOT to move it.** ~**10.3 GiB** measured under load at 53 min uptime, **up from 9.2 GiB** shortly after warm-up (it grows; n=2, plateau unmeasured) — so GPU 0's 11,982 MiB free is a **1.7 GB margin and shrinking**, on the live chat serving path. ⚠ Two measurement traps: it reports **nothing at idle on the wrong card** (`BREEZE_GPU_DEVICES=0` = the **3090**, not the A6000), and an early reading understates it. ⭐ The real objection is **topology**: `tts-gateway` is on irv-ml1 and reaches it same-box, so moving breeze alone adds a cross-site hop to every TTS call against a 478 ms first-sample budget. GPU 3 would fit it but spends the reserve. → `persistent-memory.d/2026-09-15-breeze-placement-sizing.md`

## Tried and abandoned

- `[2026-10-06]` **Restarting a user-facing service after an idle check that only PRINTS** — `nvidia-smi … | wc -l` said 1 and the same command restarted Scriberr anyway, killing Prime's transcript run. Make the check gate (`&&` / exit). auto-memory `feedback_idle_check_must_gate_the_restart`.
- `[2026-10-06]` **Aging PBS snapshots by directory mtime** — verify jobs and notes edits rewrite `index.json.blob`, so a 10-04 snapshot read as 10-06. Age by the snapshot NAME. auto-memory `reference_pbs_snapshot_mtime_moves_on_verify`.
- `[2026-10-06]` **`deploy-stack.sh` on a stack whose host dir holds rollback copies** (albok-service `compose.yaml.pre-*`) — its rsync `--delete` removes them. Upload the single changed file with elway instead.
- `[2026-10-05]` **Sizing a vLLM seat's KV from `--gpu-memory-utilization` on a shared card** — co-booting neighbours corrupt the profile (non-torch −0.56 GiB), so the KV is a lottery per boot. Pin `--kv-cache-memory`. auto-memory `reference_vllm_util_kv_coboot_race`.
- `[2026-10-05]` **Reading GPU headroom just after a restart** — 10-01's "Free ~1.05 GB" was really a ~60 MiB plateau 2 days later. Size against Beszel history. auto-memory `feedback_vllm_headroom_at_plateau_not_boot`.
- `[2026-10-04]` **`apt-get` run in-band through `qm guest exec`** — qemu-guest-agent was in the upgrade set, restarted itself, and systemd killed apt mid-dpkg on BOTH PBS VMs. Run apt in a `systemd-run` unit. (auto-memory `feedback_no_inband_apt_through_guest_agent`)
- `[2026-10-04]` **Pulling files with `ssh host 'sudo cat …'`** — sudo ≥ 1.9.14 truncated the piped stream at exit (Debian 13 hosts), and `log_output` re-recorded the whole file as a new session. Read without sudo (group-readable file).
