chore(graphify): add Granite-labeled codebase map

graphify-out/GRAPH_REPORT.md: god-nodes + call topology, labeled via local Granite-4.1-8b ($0). Heavy artifacts gitignored; rebuild: 'graphify update .' then 'graphify label . --backend granite'.
